AFFIDAVIT OF DISCREPANCY
I, ELIZABETH ARANETA UTTOH, of legal age, married, Filipino
citizen and a resident of Zamboanga City, under oath, do hereby depose
and state:
That I am a pilgrim and currently processing my documents
relating to the said pilgrimage. Hence, it is SHEIKH ALJINUL Y.
ABDULLA who is the one processing such;
That last June, 2015, he paid a Mutawiff and Vaccination fee ,
unfortunately, he paid the amount of Two Thousand Three Hundred
Fifty Pesos ( 2,350.00) fee and One Thousand Seven Hundred Thirty
eight dollars( $ 1, 738.00) ;
That however, one of the bill payment slip made by the
Sheik which was issued by the PNB Bank, my family name
appearing in the said slip is DINGLE my married name
contrary to my passport maiden name as UTTOH
That DINGLE is my married name per Certificate of
marriage and UTOH is my maiden name per Certificate o Live
Birth;
That ELIZABETH ARANETA UTTOH and ELIZABETH
UTTOH DINGLE both refer to one and the same person;

That JIMMY U. JULKIPLI and JIMMY C. JULKIPLI

appearing in both slips dated June 11, 2015 with different
corresponding time, 16:17:13 and 16:08:20 both refer to one
and same person; (Attached herewith are the PNB Slips)
That I hereby affirm that the names JIMMY U. JULKIPLI
and JIMMY C. JULKIPLI both refer to one and the same
person;
That I am executing this affidavit to attest to the veracity
of the foregoing facts and for whatever legal purpose it may
serve.
