AFFIDAVIT
I, KUMAR ASHOK SHARMA married to IRIS SALVADOR SHARMA,
both of legal age, married, Indian National and Filipino, with residence and
postal address at Dipolog City, under oath, do hereby depose and say:
That we are validly married on July 25, 2013 at Dansulyan Palace,
Zamboanga del Norte.
That however, in our Marriage Certificate, my Citizenship and Residence
address was erroneously entered as Filipino including my residence address at
Dansulyan, Polanco Zamboanaga del Norte;
That because of such discrepancies we are on the process of correcting
such entries following legal remedies as it may serve to our best interest;
That the discrepancies appearing in our marriage certificate was cause
by excusable negligence on their part and was not intentionally done;
That I am executing this affidavit to attest to the veracity of the foregoing
facts and for all legal intents and purposes this may serve.
