Phoenix Pride Award

In order for students to demonstrate going beyond what is required in class, they must complete 3 goals in
each area below. Please document with notes if needed, to show completion of each goal. Parents: Your
initials and a date need to be marked as each goal is completed. Please return the completed form by May
23.

1. Walk/run 200 laps on field at school and chart your progress.

2. Have Mrs. James sign off that you were an active participant in PE this year.
3. Be able to run a mile in 10 minutes and 30 seconds.
4. Demonstrate and teach a PE game to your class.
5. Participate in a team sport during the school year.

The Arts ...

_____ 1. Share a musical or artistic talent to the class.
_____ 2. Attend a fine arts program...save program and write 1 paragraph response.
_____ 3. Create a 5-minute play or puppet show; perform it for the class.
_____ 4. Be a participant in the Inspirations Arts contest.

History & Science...

1. Earn 100% on States and Capitals Test.

2. As a study of US history, prepare and share a report on a US president of your choice.
3. 90% or higher on 5 History and/or Science Tests.
4. Choose a unit of science that you can learn further at home about and share with the class.
5. Recite the Preamble of the Constitution.

Literacy & Math...

_____ 1. Read 20 books on your own this school year...make a list of the books.
_____ 2 Pass of all multiplication and division fact tests.
_____ 3. 85% or higher on Four Math tests.
_____ 4. Earn 100% on 15 Spelling tests.
_____ 5. With a book of your choice, do a book report of any kind and turn it in by May 1.
