6th Annual Hooked on Life-Skills Charity Bass Tournament

(PLEASE READ CAREFULLY BEFORE SIGNING)
In signing this agreement, I hereby release Life-Skills, Inc. and The Town of Webster, its officers,
agents, employees, members and sponsors from any and all damages, claims, demands, costs or expenses
relating to injury of any persons or property which I may sustain or which I may cause by reason of
participating in or in connection with the 6th Annual Hooked on Life-Skills Charity Bass Tournament
(10/18/15). I will abide by all tournament rules listed above and release all interpretations of these rules
to the tournament committee. I also certify that I carry a minimum of $300,000 liability insurance on the
boat used in the 6th Annual Hooked on Life-Skills Charity Bass Tournament.
