I Ammaji Musunuru after being duly sworn do hereby depose and say that:
1. I am a citizen and national of India born on Aug 15 th 1955 at Agiripally Krishna
district, Andhra Pradesh, India reside at Plot #673, Hmt Hills, Kukatpally, Hyderabad,
Telangana, 500085, India.
2. I have direct knowledge of the birth of ChandraSekhar Kadiyala, as I am his Aunt
and was present at birth.
3. ChandraSekhar Kadiyala was born on Nov 1st 1981 at Hyderabad, Telangana, India.
4. ChandraSekhar Kadiyalas natural parents are deceased ShanthaKumari Kadiyala, the
mother and NageswaraRao Kadiyala, the father.
5. This Affidavit is being submitted in support of the birth facts and parentage of
ChandraSekhar Kadiyala.
6. It is my prayer that this Affidavit be accepted for this purpose.
