    If the Proficiency Testing Provider is part of a larger organization, enclosed a
    chart of its position and reporting relationships within that organization.
    Enclosed a completed relevant TNI PT and or SSAS Provider checklists.
    Enclosed a copy of the PT/SSAS Providers current uncontrolled quality manual,
    operating procedures and work instructions. Electronic copies are preferred.
