Maria Mercedes on December 22, 2014, I certify that she is
suffering from hematoma and ruptured big blood vessel in the
liver by reason of blunt force inflicted to the abdomen. That I have
attended the patient for 3 weeks in the hospital to observe the
said wound operated upon, and I consider a period of absence
from her work of 3 months absolutely necessary for restoration of
her health and avoid the wound operated upon to open up.

Mercedes on December 23, 2014, I certify that she is suffering
emotional and psychological trauma is the result of extraordinarily
stressful events that shattered her sense of security, making her
feel helpless and vulnerable in a dangerous world. Traumatic
experiences involved a threat to life or safety, which left her
feeling overwhelmed a, even if it doesnt involve physical harm to
her.

PSYCHOLOGISTS FINDING

I, Colleen C. Casinabe, has examined Juana M. Mercedes, and

saw that the latter is unable to handle intense feelings, and
continues to experience strong distressing emotions. She also
shows symptoms of being unable to talk, agitated and on the
edge. She has continues to have nightmares and has no one to
support her, whom which can share her feelings and emotions.
It is advised that the child shall be under counseling to
relieve her of anxiety and fear, and to maintan her normal daily
routine. In order to relieve her of psychological distress, she shall
be kept away from his father, the source of such distress.
