Identifying Information

Miss. A.B.C is a 14 years old single female. She is 2nd born among 1 brother and 2 sisters. She is
the student of 9th class. Her father’s education is M.A and he is an electrical supervisor by his
profession. Her mother’s education is M.Sc and she is a house wife. She belongs to an Urdu
Speaking, Muslim family with middle socioeconomic status. She lives in a nuclear family set up
and resides at Model Colony, Karachi.

Referral Source and Presenting Complaints

Miss. A.B.C was referred by Dr. Raza-ur-Rehman (Psychiatrist at DMC & Civil Hospital,
Karachi) to the Institute of Clinical Psychology, University of Karachi for the purpose of
psychological assessment and psychotherapy. His presenting complaints include; Severe pain in
whole body, especially in head, knee, foot and hands, nausea, unable to maintain her bodily
balance, low mood, crying spells, can’t maintain eye contact and disturbed sleep.

Behavior During Assessment Sessions

Throughout testing sessions Miss. A.B.C was relaxed and comfortable with the examiner. She
provided information about hers problems openly. All the testing sessions she was late almost 30
minutes but she followed the instructions given to her by the examiner in the assessment
sessions.
Psychological evaluation

On neuropsychological screening test (BG) client’s score indicate that her visual motor
perceptual functioning fall in “High Average” range.

Her score on Standard Progressive Matrices (SPM), a test of nonverbal abstract reasoning (i.e.
one aspect of intelligence) indicates that Miss. A.B.C’s intellectual capacity falls within the
“Defective” range.

Projective analysis reveals that Miss. A.B.C's self image is negative. Her prominent needs are
autonomy and aggression. She perceives her external environment as stressful, threatening and
unsupportive. Due to which she feels insecure within her environment she is unable to express
her emotions to other people. It shows that her current environment is not favorable and she
perceives it unsupportive. Because of these she is maladjusted in her environment. Projective
analysis further reveals that she is inflexible and has difficulty to accept to change in her
environment. All these leads to depressed mood. Her significant conflict as revealed through
projective tests are intimacy vs isolation and intra vs extra aggression. In order to deal with her
anxieties and conflicts she uses the defense mechanisms of ‘Isolation’, ’Denial’ and
‘Rationalization’ but remain unsuccessful to overcome these.

Projective analysis further reveals that she is overemphasizing the concrete and obvious aspects
of situation. She is expends less effort than needed or required to process information adequately.
Client has low level of empathy and a withdrawal from interpersonal relationship and she has a
stereotype manner of approaching the world. She is more active in term of thought and she is
more receptive to emotional input and more likely to respond immediately rather than delay
behavior. Further she impulsively take decisions and is unable to consider all relevant aspect of
situation.

Tentative Diagnosis

300.11 Conversion Disorder, With weakness or paralysis, With psychological stressor

Recommendation

• Cognitive-behavioral psychotherapy strategies may be specifically helpful in reducing


distress.
• Psycho education can be helpful by letting her know that physical symptoms may be
exacerbated by anxiety or other emotional problems.
• Work collaboratively with the client investigating thoughts, feelings and behaviors
together and agreeing on what changes to make.
• Deal effectively with stress through stress management techniques
• Use structured learning experiences to teach the client to monitor and write down her
negative thoughts and mental images. The goal is to recognize how those ideas affect her
mood, behavior and physical condition
• Teach the client ways to cope with conflicts and personal difficulties.
• Help the client in restructuring her thinking patterns and behavioral activation and guide
her to overcome obstacles to participating in enjoyable activities.
