III. Case Background:

The client came here with her father for a psychological assessment on August 19th,
2019. This has been their third time visiting NCMH, with the two previous being check
ups. According to the father, A.M. does not have a mental illness and that she has not
been confined in a psychiatric hospital before. She was referred by the Makati DSWD to
get a psychological assessment because according to her father, she is "distorting reality"
and replaces the biological father's father identity with that of the mother's boyfriend. The
father told us that he caught the boyfriend fetching the client and her mother and they
would go to the boyfriend's condo and that the three of them sleep in a single bed. Her
father also told us that A.M.'s mother shouts a lot and the client was once hit by a hanger
by the boyfriend.

She has neither auditory nor visual hallucinations. She also has no problems with her
sleep and her appetite is normal. She is not taking any medication whatsoever.

IV. Testing Behavior

The client had her hair in a messy braid with two black hair clips on the front. She was
wearing a printed white shirt, printed orange leggings, blue rubber shoes, and white ankle
socks. She is quite chubby and she has rosy, fair skin. At the time of testing, she brought
a full leather pastel pink backpack filled with snacks and her other stuff.

The client was talkative and she was sharing stories as we were testing. She was also
curious with some of the tests and would ask us questions regarding those. Her preferred
speaking language was English and she was very fluent at it however she still
understands Tagalog. Her voice is small and feminine and she has somewhat of an accent.

She maintained good eye contact all throughout. She looked into our eyes directly
when we spoke to her. She was in a good mood and she was very cheerful. She was also
attentive and quick to respond. She was aware of her surroundings and what was
currently happening. She was nice towards us examiners, as she would even share her
food with us. She was cooperative and we were able to go through the whole process
smoothly.

V. Test Evaluation

WISC-R Verbal Scale - 114

Performance Scale - 112
Full Scale - 115
Classification - High Average

A.M.'s current intellectual functioning is at the High Average classification. The results
indicate that she has better verbal abilities than her performance abilities. She has
superior visual motor speed and she is able to see the relationships between things and
perceive the relationship parts of a whole. She has above average long term, short term,
and visual memory along with numerical ability and learning ability and has average
social judgement, social intelligence, and deductive reasoning capacity.

Based on the results of the projective tests, the client has emotional problems, having
trouble with emotional control which have stemmed from the conflict between her
parents. It also says that she has feelings of anxiety, as indicated by her wanting to run
away from home when she gets scared.

In terms of social functioning, the results suggest that she is hypersensitive to social
opinion, as she is bullied at school. She may have a maladjustment issue and that she

desires interpersonal contact (most likely from mother). She expresses a strong desire that
her parents get back together. Additionally, it shows that she enjoys being in authority
and leading her classmates at school.

Regression is the main defense mechanism that stands out in the test results. This is
evidenced by her wanting to get her toy during testing. Other than that, she has
aggressive tendencies which may be again supported by the bullying she faces at school.

VI. Overall Impression and Recommendation

Overall, the client is having emotional problems, particularly in emotional control

which can be traced from the issues from her parents plus the possible bullying she faces
at school. It is recommended that the client seek family counseling to address parents'
issues for the client. It is also recommended that the child receives intervention at school
with regards to bullying.
