Diagnosis

Justification

Fluid volume deficit related to


uncontrolled diabetes insipidus as
evidence by urine output 4300ml
and intake 2150ml.
Activity
intolerance related
to fatigue and frequent urination as
manifested
by
weakness
and fatigue of the patient.

According to Maslow, the fluid is


physiological needs. Also fluid
volume deficit can lead to
dehydration.
Most activity intolerance is
related to generalized weakness
and debilitation secondary to
acute or chronic illness and
disease.
This
is
especially
apparent in elderly patients with
a
history
of
orthopedic,
cardiopulmonary, diabetic, or
pulmonary- related problems.
The aging process itself causes
reduction in muscle strength and
function, which can impair the
ability to maintain activity.

Risk of impaired skin integrity


related to Urinary frequency with
high volume output and the
potential
for
incontinence
as
evidence by urine output 4300ml

This is the 3rd Prioritized nursing


diagnosis because according to
the rule risk problem should
prioritized least, because actual
problem should solve first.
