The Cabarrus County Senior Centers, an affiliate of the Cabarrus County Department of Aging, is not
only a place for older adults come together to receive services, but also provides a multitude of home
services that benefit the members of the community that are 60 years of age and older. The Department of
Aging coordinates several human service programs funded by the Older Americans Act, which offer
overall wellness activities, promote health and/or delay any decline in health status. The Department
encourages an active, healthy lifestyle for older adults by promoting total wellness and allowing our
participants to remain independent. This philosophy preserves the dignity of each individual while
minimizing institutional care. They also coordinate efforts with other agencies to assist older adults in
receiving the services and support they need and desire (http://www.cabarruscounty.us/Aging/). This
service is not located in the community that is being assessed but its services can be utilized by all
residents of Cabarrus County. These services include:

Adult Day Care is available for residents of Cabarrus County who are 60 years old or older. The
services are provided by a certified Adult Day Service Center located in the community. Adult
Day Services are designed to meet the daytime needs of participants who have physical and/or
memory related disabilities, or who live at home and depend on support from a caregiver. These
services promote independence by allowing the participants to continue to live at home because
daytime supervision is available while the primary caregiver is able to work outside of the home.
Respite assistance is also provided. Personal care, meals, health screenings, recreational/leisure
activities and rehabilitation services may be provided.

This program provides educational and training opportunities and respite for caregivers that will
assist them with the necessary knowledge and resources to provide the level of support that their
loved one deserves. It also provides access to available services that will help them meet the
diverse needs for their specific circumstances. Services may also be available for grandparents
raising grandchildren.

In±home aides are available to provide temporary relief to 24±hour caregivers who are caring for
a loved one who cannot be left alone due to physical or mental impairments. To receive this
service, either the caregiver or the care receiver must be at least 60 years old.

This organization provided meals to residents of Cabarrus County who are unable to prepare and have no
one to prepare one hot balanced meal per day. They also provide frozen meal for weekend in addition to
the weekday hot meal delivery service. In order to receive this service, the recipient must live in Cabarrus
County, no ability to prepare one hot meal per day, need the service for a minimum of three weeks and
live within a Cabarrus Meals on Wheels delivery route. Payment is based on a sliding scale based on
income and sponsorship is available for those who are unable to pay. Meals on Wheels also can provide
assistance with pet food for those who receive food services and cannot afford to feed their pet.

The end of life community in Concord is served by Hospice and Palliative Care of Cabarrus County, an
affiliate of CMC-Northeast. Hospice & Palliative Care of Cabarrus County has agreements and
partnerships with a community of healthcare providers to offer comfort and improve quality of life in the
final weeks and months of life. Hospice services enable patients to remain in their home and received care
that in the past was only available in the hospital. Collaboration with physicians, hospitals, nursing homes
and health insurers make Hospice care available to everyone who wants it in Cabarrus County. In order
to receive hospice services, a patient must have a life limiting illness with a diagnosis of six months or
less to live; care focused on comfort and not cures and resides within the service area. Hospice services
may be utilized in the home, skilled nursing facility, nursing home, assisted living center and the hospital.
Hospice is a Medicare /Medicaid benefit and can be covered by private insurance. The patient in
conjunction with the family and interdisciplinary care team determines the care needs and the services to
be utilized.Y

Home Visits (CNA) to provide supportive care to patients and families,
bathing services, and personal care. The CNA is in contact with the nurse and communicates any
changes or information pertinent to the patient¶s care.

attends to the patient and family¶s non-medical concerns that can
develop during an overwhelming time. The social workers provide for the emotional needs of the
patient, family, and caregiver. In addition, they provide problem solving related to additional
practical issues such as sitter and/or respite for the caregiver. The social worker also can provide
information regarding funeral arraignment, living wills and advanced medical directives,
healthcare power of attorney, legal assistance, and community resources for transportation and
financial assistance.

Concord Place is an assisted living center for senior citizens. Each resident has a private apartment and

Located in Concord provides rehabilitations, nursing and assisted living services. The rehabilitation
includes physical, speech and occupational therapy. The staff consists of a Board Certified Gerontologist,
Geriatric Nurse Practitioner, Nutritionist, Registered Nurses (RN), Licensed Practical Nurses (LPN¶s) and
Certified Nursing Assistants (CNA).

The Cabarrus County Transportation Service (CCTS) provides transportation for the elderly and low-
income residents in the community. The service operates during normal business hours, five days a week.
Interested individuals should check with either the Cabarrus County Department of Social Services, the
Department of Aging or LifeSpan Inc. for eligibility. The ¢ $
has several
different programs to assist the community and will cover the cost for their transportation. Medicaid will
cover transportation to doctor appointments and pharmacy visits (for prescription pickups only). The
¢ $ &!! assists residents sixty years of age or older who are not receiving Medicaid. The
Department of Aging will also cover the cost for transportation to doctor appointments and pharmacy
visits (for prescription pickups only). All reservations must be scheduled no later than 1pm the day before
the appointment. Reservations can be scheduled up to two months in advance.
