APPLICATION FOR A TEMPORARY RESIDENT VISA CHECKLIST

Complete and place this checklist on top of your application.


Consult our website for forms and instructions, including a list of countries whose nationals require a
visa: www.philippines.gc.ca.
Any document not in English or French must be accompanied by a certified translation.
Do not include photocopies of your application package. You may keep them for your own reference.
Failure to submit all required documentation may result in the refusal of your application or delayed
processing.
False statements or submission of fraudulent documents will result in immediate refusal.
After submission of all required documents you may be required to attend an interview or medical
exam.
Any preparations you make or expenses you incur are done entirely at your own risk. Do not finalise
travel plans unless you have received your visa from the Embassy of Canada to the Philippines.

You must submit the following:


Completed Application for a Temporary Resident Visa (IMM5257) for each individual
applying. If you and your spouse/common-law partner and dependent children are applying
together, each family member must complete and sign a separate application form. Those
applicants under the age of 18 must have a parent or a guardian sign on their behalf.
Completed Family Information form (IMM5465) for all applicants 18 years of age or over.
Correct processing fee. Fees must be paid by managers cheque, postal money order, certified
cheque or bank draft payable to the Embassy of Canada, Manila. Cheques and PMOs should be
valid for at least 6 months upon submission of your application. Cash will NOT be accepted and
processing fee is non-refundable. Please see our website (www.philippines.gc.ca) for more
information.
Original passport(s) for yourself and each family member requesting a visa.

Each passport should have a minimum of two blank visa pages and minimum 6 months
validity.

Attach old passports showing previous travel.

Children must have their own passport.


Two passport photographs for yourself and each family member requesting a visa, with the
name printed on the back, dated and taken in the past 6 months.
Explain the purpose of your trip Mention any other countries to be visited on your trip, other
persons accompanying you, and any other information supporting your reasons for travelling. For
convenience, you may use the Supplementary Information Form on our website.
Original proof of funds Updated original bank certificates with transaction history for at least
the last 3 months (in the form of passbooks, bank statements, or time deposits).
If you are going to visit family or friends, you need to provide your own proof of income
and funds to demonstrate your financial situation in the Philippines. You may also
provide the following:

A letter of invitation from your relative or friend that includes their full name, address,
telephone and fax numbers, your name and relationship to the inviter, the intended duration
of the visit, and a statement concerning who will pay for the trip expenses. For more

A copy of citizenship or immigration status document of the person inviting you (to

demonstrate that they are legally in Canada).


For the person who will pay for your trip, you may supply financial supporting documents to
prove that they can afford to support this visit, including reliable and verifiable information
from a third party source of your host's income, employment letter and proof of funds.
*Note: Some hosts/sponsors may prefer not to provide their personal information directly to the
applicant for submission to the Visa Office. In this case, please enclose a note to this effect.
Please be advised that this may lead to slower processing in some cases. The application will be
assessed based on the documentation and financial resources provided.

If you are going on a business visit, please provide:

Letter of invitation from the company you intend to visit including full name and title, business
address, telephone and fax numbers, your own name and title, the intended duration of the visit,
a brief summary of the reasons for the visit and a statement concerning who will pay for the trip.
Purpose of visit - Copies of any additional documents that outline the business to be conducted
during the visit. These may include business correspondence and proof of previous business
conducted (contracts, shipments, orders, invoices, etc.)
Contact names, titles, and addresses, phone and fax numbers for third parties with whom you
have arranged meetings.

If applicable, please also submit:


Use of Representative form (IMM 5476), if someone is assisting you in completing your
application or you would like someone to do business with our office on your behalf.
Proof of employment A letter from your current employer and payslips (last 3 months). The
employment letter should be dated within 2 months of the application and should indicate
occupation, job description, position, salary, allowances, years of employment, and authorized
leave dates.
Proof of Business if you are self employed. Provide business registration, tax clearance
certificates, trade certificates, business license, and financial documents such as certified business
bank account statement for past 6 months.
Proof of current studies A current letter and transcript from the educational institution you
are attending.
Children under 18 years of age travelling without their parents require:

a letter of permission to travel, from the non-accompanying parent(s);

a letter from their custodian in Canada;

travel clearance certificate from the Department of Social Welfare and Development (DSWD)
of the Philippines;

a school leave letter, if applicable;

if the child is the subject of a custody order, proof of custody and the other parent's consent
must also be provided.
