form. Candidates are strongly advised to read the 2016 Statute for the Degree of Doctor of Philosophy

From 1 January 2016, I wish to:

be enrolled and registered under the provisions of the 2016 Statute and Guidelines for the
Degree of Doctor of Philosophy; or

be enrolled and continue my registration under the provisions of the 2011 Statute and
Guidelines for the Degree of Doctor of Philosophy (this option is only available to those currently
enrolled under the 2011 Statute); or

be enrolled and continue my registration under the provisions of the 2008 Statute and
Guidelines for the Degree of Doctor of Philosophy (this option is only available to those currently
enrolled under the 2008 Statute); or

be enrolled and continue my registration under the provisions of the 2003 Statute and
Guidelines for the Degree of Doctor of Philosophy (this option is only available to those currently
enrolled under the 2003 Statute); or
Section B: Student Declaration

I have read the 2016 Statute and Guidelines for the Degree of Doctor of Philosophy and am aware that
my registration will be governed by the Statute I have indicated in Section A.
