Underground Utilities:
Hazardous Condition

No ladder there is no mean of access or egress and the equipment is working


in right at the edge
This work is in trench with no protective system and has no mean of egress
no signboard and barricades
The pump should be positioned where it will not create an overhead hazard
and must be monitored by a competent person. In situation such as this, the
worker may be required to wear a harness and lifelines
In case of excavations over 1.22 meter s in depth, the confined space entry
procedure will be adhered, to ensure acceptable atmospheric working
conditions, and to prevent exposure of personnel to harmful level of
contaminants i.e. LEL Oxygen H2S CO etc.

Hazardous Condition / VENTILATION

Displace hazardous gases and vapors.


Considerations

o Heavier than air or lighter than air contaminant


o Exhausting or blowing in
o Volume / time required to lower concentrations to acceptable levels

SUMMARY: EXCAVATION HAZARDS

Collapse of earth work due to lack of, inadequate, or weak shoring.


Failure to maintain shoring particularly after inclement weather.
Persons falling into excavations due to lack of Hard barriers, Signboards or
inadequate fencing, no flashing light.
Spoiled soil from excavations not being kept clear of the sides which then
become overloaded and collapsed.
Water seepage.
Person working too close together.

Vehicles or plant working too close to the edge causing the edge of
excavation to collapse.
The striking of underground services, e.g. electrical cables and gas pipe lines,
etc.
Asphyxiation by dense gases such as

H2 S

can collect in excavations with poor ventilation.
