Notes:
Shut Down: M spent the overwhelming majority of this week in "shut down" or
"destructive" frames of mind. Over 4 days I was only able to get him to complete
tasks for a total of 45 minutes.
On-Task: M is still working on 1st grade level math and reading academic tasks, as
well as social-emotional regulation tasks, and life skills. He worked with our
Speech and Language Pathologist LT to complete some Language testing. L had to
try coming back 2 times before M was able to repeat back 6 sentences to her.
Aggression: Aggression has been documented as hurting self/others, and
throwing/damaging personal and school property. With a smaller room, and a
decreased number of options to destroy, M has been going after adults more this
week than any other week, and attempting to destory more dangerous objects.
He kicked an outlet to the point of wires coming out, he went after staff by
kicking, hitting, pulling scarves and lanyards, and throwing objects at adults.
During these escalated moments M can be heard yelling, "Call my mom" and "I
hop you die," "Shut up!" "F you!"
Documentation: All of the data documented in this document has been gathered
from ABCH froms provided by our behavior specialist. These ABCH forms are
available upon request. The data will be used to continually update the BIP, and
determine the student's re-entry into the general education setting or increased
number of hours at school.
