Instructional Intervention Project Plan

Students Initials: C. M.
Age: 8
Grade: 3rd
Gender: Male
Targeted Behavior (IEP goal): By the end of 36 instructional weeks,
presented with a graph or table, the student will answer related
questions with 80% accuracy in 4 out of 5 trials.
Data Collection Form: 3 different work samples taken over the course
of 5 instructional days.
Intervention: Small group differentiated instruction with different
materials. I will explicitly teach academic vocabulary and use a variety
of activities to generalize the material.

Baseline data on Activities 1-3
After looking at the graph, you can see a positive trend line.
Once the student began receiving small group, explicit instruction, the
students grades on assignments began to improve. The student
responded the best to hands-on learning. The first 100% he received
was on an assignment where he rolled a superhero dice and created
his own graph. He then accurately answered questions on the graph he
created. The intervention was successful.
The last assignment allowed a 2-week time lapse to see if he
retained the information. The student received a 100% on the
assignment, even though we had not discussed graphs in some time.
The information he learned was generalized and put into long-term
memory.
This project showed me how crucial differentiating lessons is. The
student did not respond well to typical whole-class instruction. He did
better with kinesthetic and visual learning. I realized that all students
really do learn differently, and it is my job to make sure that they are
receiving the instruction they need.
