DEVELOPING A PROJECT
I discussed possible options with supervisor.
She stated that although the discharge packet
gives examples of types of community resources,
she did not have a list of resources in Robeson
County to give discharging residents.
We decided I would put together that list.

DATA COLLECTION
The social worker keeps an business card index in
her office that has relevant and useful resident
resources in it.
She sorted through them and told me which ones
were relevant for the project.
I compiled a word document with the contact
information for each agency.
Later, I sorted the list into categories, and I added
a few resources at the end I felt might be useful
(such as the library and senior center).

WHAT IS INCLUDED
The packet included:

Assisted Living Facilities

Transportation Services
CAP Program
Meals on Wheels
Lumberton Library
Pine Street Senior Center
North Carolina Elderly Psychiatric Services (NCEPS)
NC Department of Health and Services for the Blind

HOW IT WILL BE USED

The project is going to be included as a document
in all discharge packets.
The clients will then have necessary information
on community resources. For instance, if the
client were to need wheelchair accessible
transportation.
