DEMOGRAPHICS

Age: 71
Gender: Female
Race: African American
Type of Care: Long-Term

REASON FOR PLACEMENT

The resident initially came into the


facility because she had an infection
that caused her to lose both of her
legs. She stated that when she came in
she was out of it, and she cannot
remember the time period of her initial
placement. Now, she stays because
she requires assistance with daily living
and does not have the ability to go
home.

STRENGTHS AND LIMITATIONS

Strengths:

Alert and oriented


Able to retain information
Participates in facility activities
Interacts with staff and other residents
President of the Residents Council

Limitations:

Can be manipulative/non-compliant to get out of


therapy or doing something on her own
Not much family involvement

INTERVENTION

Social Services:

Basic Interview for Mental StatusAssesses


cognition level. Resident did not require
intervention on this level.
Resident Mood InterviewAssesses mood.
Resident did not require psych interventions for
this assessment, but social worker stated the
resident is already followed by psych for anxiety.

Therapy

Interventions have been done over the years


(diathermy (for pain) and strengthening).

STAFF RECOMMENDATIONS

Social Worker did not have any


additional recommendations.
Therapy continued to recommend the
diathermy, pain medicine, and
independent exercises.

I do not recommend any changes.

FROM THE RESIDENT

She says that her mom shaped her into who she is
today. She stated that her mother taught her
everything and was a good mother.
She states that she likes the atmosphere of the
facility. Once she left in order to be closer to her
family, but she stated that she did not like the
way the pother facility felt and that it wasnt like
home.
We did not have any conflicting values/biases due
to the fact the assessment does not cover that
information.
