History of present illness:
JKB, 31 years old married female belongs to the urban area lived with her
husband presents to the clinic with chief complaints of disturbed sleep and
appetite from past 1 month. According to her she wakes up in the night
and finds is difficult to sleep again. She also notices less appetite and
weight loss. She also believes that someone has done black magic on her
and on her husband which causes all the problems.
History of Past Illness:
According to the client she had no significant past history of
psychological illness.

Past Medical History:


Not Significant

Birth and Developmental


History:
Normal/all the developmental milestones were achieved
within normal limits.
Behavior During Childhood:
According to the client she was a very happy child. She
liked to play with her friends.

Family History:
According to the client she is the elder one of three siblings and has
one younger brother and sister. She loves her sister so much.

• According to the client she is not sexually active.

Educational History:
• The client completed her Mater’s in Fashion Designing one
year ago after her marriage. She mentioned that her husband
is so supportive and helped her a lot in pursuing her dreams.
Pre - Morbid Personality:
• Attitude towards self and others: The client thinks she is not a
good person now, because of her everyone suffers in family. She doesn’t like
to share her feelings and thoughts with anyone in the family. She was socially
active in school or college.

• Moral and Religious Standards: The client mentioned that


she believed in god so much. She belonged to a religious family.
