If an athlete wishes to travel to or from a game site apart from the team, with a parent or
guardian, the form below must be filled out. This form must be in the hands of the coach the
daybeforetheeventorgame.

We are a team and will show our support by remaining at all gamestocheeronourteam. We
will learn from each other and learn how to act like a team by sticking together, whether in
victoryordefeat.

By signing this form, I understand that student athletes are required to go and return from
athleticcompetitionsonthetransportationprovided,unlesspriorarrangementshavebeenmade
andagreeduponinwriting.
