Gingiva: Focally, the gingival epithelium is ulcerated. The underlying stroma is replaced by fibrous
connective tissue with interspersed perpendicularly oriented blood vessels (granulation tissue), frequently
separated by clear spaces (edema). This is focally overlaid with numerous neutrophils mixed with fibrillar,
eosinophilic material (fibrin). Focally, the stroma is effaced by hemorrhage.
HISTOLOGIC DIAGNOSES:
1. Gingivitis, focal, suppurative, moderate, chronic, with granulation tissue
COMMENTS: No specific cause for the gingival ulceration is evident. Based on the presence of
‘granulation tissue, the lesion has been going on for a period of weeks. The neutrophils indicate an active
process; tis is likely secondary to resident oral bacteria.
