Addendum I International study sites

The following questions provide the IRB with information about the international
research site where the research will be conducted. These questions may be
answered by the study investigator or any other individual with appropriate site
knowledge. If another individual is used for site expertise, provide the completed
application to the expert so that he or she can answer these specific questions.
Questions answered by:
Study Investigator or team member (include name, experience with study
location, and total time spent working on site): J. Luis Guardado is in charge of
the data collection and other aspects of the project (e.g., providing
informed consent etc). Luis is native from El Salvador and is very familiar
with the culture, language and school system. He also has been
volunteering in the school for the last few weeks, holding motivation and
memory workshops for teachers.

1. Do you foresee any potential physical, psychological, social or economic risk to
research subjects who would participate in this type of research? This study may
involve some degree of anxiety or uneasiness that in most cases is negligible; the
experimenter, teacher and/or any other person proctoring testing sessions will be
instructed to pay special attention to any discomfort produced by the test. There
may also be other risks that we cannot predict although we do not have any
reason to think other risk exists.

2. Would any of these individuals be in jeopardy for providing the investigators with
their opinions on this topic? No individual would be in jeoparly for providing
feedback or opinions to the PI or other people involved in the project.

3. Are there any specific regulations that you know of for the conduct of research in
this area of the world? We are not aware of any regulation out in place in El
Salvador.
