AFFIDAVIT
I, LIGAYA D. ROSEL, of legal age, Filipino, widow, and a resident of #13A
Buli, Muntinlupa City having been duly sworn in accordance with law,
hereby depose and state that:
1. I am one of the surviving siblings of Atanacia De Chavez Cabaeros;
2. My deceased eldest sisters last name is Cabaeros as stated and
registered in her death certificate issued by office of the Civil Registrar
General. A copy of her death certificate is hereto attached as ANNEX
A;
3. The person Atanacia De Chavez Caballeros appearing in the
document which I submitted to Allegro Microsystem Inc. and
Atanacia De Chavez Cabaeros refer to one and the same person;
4. I am executing this Affidavit to attest to clear any discrepancy that
arises in my records, to explain the discrepancy and for whatever legal
purposes it may serve
