Final project
• Our patient is a 25-year old divorced mother
of two children works as an accountant and
lives with her parents. She was diagnosed with
HIV infection during the pregnancy three years
ago. She tires easily but is able to work. She
reports bouts of depression and anxiety.
▪ Depression, patient has too many problems
and her future is uncertain.
• ▪ HIV Infection is an illness that has not cure.
• ▪ Tires easily due to HIV attacking her immune
system.
• The problems that our patient is facing are:
• Goals:
• ▪To alleviate her depressed state within a 30-90 day
period and to reevaluate every 3-6 months.
• ▪Our patient will enter in a program for HIV patients,
where she will learn how to understand and deal
with her illness. It will help her to accomplish her
daily activities without difficulty in 3 months.
• ▪Patient’s tiredness will be under control within 3-6
months.
• Interventions:
• ▪The nurse will have her keep a log of her daily activities to
find out what is causing the depression. The nurse will
monitor and maintain her mental well being through group
support, family support, and sharing about daily living.
• ▪The nurse will give information to the patient about nearest
community center for HIV patients, where she can obtain
proper treatment and interdisciplinary personal care plan.
• ▪To reduce tiredness patient was encouraged to get at least
6-8 hours of sleep and exercise 3 days a week for 30 minutes.
• Rationale:
• ▪Group therapy is needed so she can talk
about her issues with others to see it this will
make a substantial improvement.
• ▪Giving information about the community
center for her condition will assist her to get
into an individual treatment and incorporate it
to her daily routine.
• ▪Getting adequate rest and exercise will
alleviate tiredness.
• Evaluation:
• ▪The goal was met. Patient states after 3
months attending support group 2x a week to
talk about her illness and deal with her daily
activities her problems are improving
physically and emotionally
