FOLLOW-UP NOTE

Sharon has now completed 9 of 12 infusions of FOLFOX. She has had increasing
problems with neuropathy. The last treatment induced more cold, induced neuropathy
<___> in the past. She is also started to have trouble picking up small items. She is
still able to <___>. She does not have painful neuropathy. The reduction in 5-FU was
able to diminish the problem of diarrhea and she usually only has one loose stool a day.
She has some increased fatigue that has lasted for about 10 days. Fortunately, she has
not had myelosuppression or infection.

She weighs 96 kg. Skin is warm and dry. No rash is noted. No stomatitis is evident.
Vital signs are stable. She is afebrile.

Labs reviewed and are acceptable for treatment. It appears to me that she is having
increasing neurotoxicity. I am going to reduce the dose of oxaliplatin today. If she does
well then we will complete her treatments on December 27, 2010 then began quarterly
followups.
