I hereby state that my husband sri L.Mahalinga applied for an unsecured

Loan of Rs.50000/- With THE BHARATH CO-OPERATIVE BANK
LTD.,mathikere,Bangalore.560054. and I am willing to offer myself as a surety to the loan of sri
L.Mahalinga.

I am doing… Teaching profession(tution) for childrens in my residence at

malleshwaram and tailoring Buisness. My monthly income from the above sources is Rs…
10000/-.

I hereby state that I am carrying the Teaching profession and tailoring

Business at my residence in malleshwaram, Bangalore.

My annual turnover from the above business/profession is about

Rs.120000/-. And my income is about Rs.120000/- P.A and I am not an income Tax assessee.

I hereby state that I carry in my business/profession without the

assistance of………………………………….(hire,labour,employee) of like nature.

I hereby state that apart from the above income, I have no income from
other sources.
