Liability Release Form: Vegas Nights

my own actions and behavior and do not hold the party hosts
accountable for any misconduct on my behalf or anyone else’s behalf
at the party on March 19th, 2011 at the Alamo Women’s Club, 1401
Danville BIvd. I understand there is a zero tolerance for any alcohol
or illegal substances at this event. If I decide to go against this
contract and be under the influence of alcohol or any illegal
substances I know I will be at my own personal risk and liability. I
know I am held completely accountable for my own actions, behavior,
injuries, and loss or damage of items. Without this contract signed by
not only me but as well as my parent/guardian, I understand I will not
be allowed into this event.
