RELEASE WAIVER AND QUITCLAIM

KNOW ALL MEN BY THESE PRESENTS:

That I, WILMER C. BAID, Filipino, of legal age, single and resident of Dahican, Mati City, do
by these presents states that-

1. I am previously employed at WONDERLAND AND PHARMACY, Dahican , Mati City which is

owned by ERWIN M. MALALIS.
2. I was listed as one of the recipient of monetary award in an Order which has become final and
executor y directing Mr. Malalis to pay me and other complainants the salary differential 13 th
month to pay and night differential although our inclusion was without our consent.
3. I understand that the total amount of award for me is 50,034.40.
4. I have no ax to grind against Mr. Malalis but on the contrary, I have received favors which cannot
be equaled or surpassed with any amount.
5. I do hereby quitclaim, release discharge and waive any and all actions of whatever nature,
expected, real or apparent, which I may have against Mr. Erwin M. Malalis by reason of or
arising from my employment with him.
6. I will institute no legal action whether civil, criminal, labor or administrative against Mr. Erwin
M. Malalis. Any and all actions which I may have commenced either solely in my deemed and
considered voluntary withdrawn by me and will no longer testify or continue to prosecute said
and action.
7. I declare that I have read this document and fully understood its contents. I further declare that I
voluntarily and willingly executed this release. Waiver and Quitclaim with full knowledge of my
rights under the law.
