That I, Juan dela Cruz, of legal age, Filipino, married, a resident of 334 Barangay
Banggolo, Lilod Madaya, Marawi City, Lanao del Sur, Philippines, do hereby named,
constituted and appointed John Doe, of legal age, Filipino, married and a liason officer of
Land Transportation Office (LTO), Butuan City, to be my true lawful Attorney-in-fact, for
me and in my name, place and stead, to do and perform the following acts and things, to
wit:
1.) To claim and/or receive my new Pag-IBIG Cash Card;
2.) To follow up on my behalf any documents and/or transactions in connection with
Pag-IBIG;
3.) To act in my behalf on any matters relating thereto.
GIVING AND GRANTING unto said attorney-in-fact full power and authority to do and
perform all acts and things whatsoever required and necessary to be done in connection
with Pag-IBIG as fully to all and legal intents and purposes as I might and could lawfully
do as if personally present. Hereby ratifying and confirming all that my said attorney-in-
fact lawfully do and cause to be done by virtue of these presents.
