RELEASE, WAIVER AND QUITCLAIM

after first being sworn in accordance with law hereby depose and state that:

1. I am an employee of Agrani Motors Inc., a corporation duly organized and existing

under the laws of the Philippines with business address at Pioneer House, 108 Paseo de
Roxas St., Legaspi Village, Makati City hereinafter referred to as the “Agrani Motors Inc.”

2. I hereby reiterate and confirm the receipt of my last pay and 13 th month pay in the total
amount of PESOS: P 4,345.05 the final and full settlement of any and all claims which I
am entitled under existing law, the Company’s Retirement Plan or any agreement or
contract incident to, or arising from my employment or cessation of the same.

3. I release, remise and forever discharge the Company, its successors in interest,
stockholders, officers, directors, agents and employees from any action for sum of
money, damages claims and demands whatsoever which in law or in equity I ever had,
now have or which I, my heirs, successors and assigns, hereinafter may have upon or
by reason of any matter, cause or thing whatsoever, up to the time of these presents, the
intention hereof being completely and absolutely to release the Company, its successors
in interest, stockholders, officers, directors, agents and employees from any and all
liability arising wholly or partially or directly from employment with the Company.

4. I will not, at any time and in any manner, directly or indirectly disclose to any person or
entity any information of any kind relating to the business of the Company, its manner of
operations, plans or data of any kind without its express written consent.

5. I acknowledge that I have received all amounts that are now or may be due from the
Company and that during my employment, I have received and was paid all
compensations, benefits and privileges to which I was entitled under the law and the
Company’s policies by reason of my employment therewith and if I hereafter be found in
any manner to be entitled to any amount, the above consideration is a full and final
satisfaction of any and all such undisclosed or unknown claims.

6. I declare that I have read this document and have fully understood its contents. I further
declare that I voluntarily and willingly executed this Release, Waiver and Quitclaim with
full knowledge of my rights under the law.
