This offer is limited to one rebate per product purchased. A limit of ten (10) rebate claim submissions per person or address. LG Accessory rebates cannot exceed the purchase price of the
product. In the event the original purchase price of a product is lower than the LG rebate, LG reserves the right to pay a rebate amount equal to the purchase price. Qualifying Models must
be purchased between October 3 and December 31, 2010. All forms and requested materials must be postmarked in one mailing within 60 days of the purchase date (“Rebate Submission
Period”). Late submissions WILL NOT be honored. LG Electronics MobileComm, U.S.A., Inc. (“LG”) reserves the right to audit any claim and to nullify any claim that cannot be
substantiated. LG Electronics MobileComm U.S.A., Inc. (“LG”) reserves the right to amend the rules during the promotion period or cancel this promotion without prior notice. Where
required by law, if form and materials are submitted within the Rebate Submission Period but are incomplete, a written notice of the incomplete request will be provided to the consumer and
the consumer will be given a 30 day period to correct and re-mail in the complete submission. Valid only in the U.S. and Puerto Rico, offer void where prohibited, taxed or restricted by law.
Certificate and copy of receipt become the property of LG. This rebate is only available to end-user consumer purchasers. Please allow 8-10 weeks for processing. Any attempts to
fraudulently obtain the rebate will be void. LG and its agents are not responsible for late, lost, mutilated, misdirected, undeliverable or postage due requests. Rebate offer expires if the rebate
check is not cashed within 90 days of the check date, after which time neither LG nor its agents will have any further obligation to you. Use of fictitious names or addresses could result in
