CHECKLIST & INSTRUCTIONS FOR FILLING UP CA-9 PILOT EXAMINATION FORM


As per serial number given in the CA-9 form

1. Fill the eight digit of computer number.


2. Write for the Examination Session (for example for October 2010 session write,
October, 2010).
3. Write name of candidate as given in the Computer Number application.
4. Write the Examination Centre desire to appear.
5. Check for Fee of Rs. 500/- per Paper in the form of Demand Draft drawn in favour
of “PAO, DGCA, MCA, New Delhi”, payable at New Delhi only and has validity of
6 months. The demand draft should not be more than one month prior to the last
date of examination.
6. Write the Category of Examination only one category at a time (e.g.
PPL/CPL/ATPL etc.). Write the Yes/No in the boxes given below for the papers
candidate desire to appear.
7. Write the details of Indian commercial pilot Licecce for appearing in ATPL
category
8. Sl no 8-10 to be filled by the candidate.

Note: The declaration also requires to be signed by the candidate. Application with out
signature will be liable to be rejected.
Note: Any change in the mailing address should be intimated to The Director of
Airworthiness, Central Examination Organization, O/o DGCA, EAST BLOCK III, R K
PURAM NEW DELHI-110066 in a separate application.
