How to apply for financial aid

1) Complete the Free Application for Federal Student Aid (FAFSA)


You must complete the FAFSA at www.fafsa.ed.gov to qualify for any loans, grants or federal
work-study. It is very important you choose NYCOM (code E00546) during the application
process. Without the correct school code we will not receive your results and will be unable to
process any grants or loans.

2) Complete the following forms


In addition to the FAFSA all students must submit the following:

 All students (including first-year students)


o NYCOM Preliminary Application for Financial Aid - enclosed
o Documentation of parental and student income if applying for Scholarship for
Disadvantaged Students
o Additional information at the request of the financial aid office

 Additionally for first-year students only


o Student Loan Entrance Interview – enclosed
o Authorization Statement -enclosed
o Title VII Supplemental Data Request form -enclosed

3) Apply for Loans


Federal Direct Subsidized/Unsubsidized Loans and Federal Direct Grad PLUS Loans
All new students and students applying for financial aid for the first time MUST complete
a Master Promissory Note (MPN) for both federal Direct Loans (Subsidized and
Unsubsidized with one MPN) and federal Direct Grad PLUS Loans at
https://StudentLoans.gov. Our school is listed as “NY Coll of Osteo Med @ NYIT”.

Students who utilized an endorser for their Direct Grad PLUS must complete a new MPN.

Failure to complete a new MPN will result in a delay of your refund. NYCOM will
not issue any emergency refunds.

Federal Perkins Loan


Unfortunately, there is no funding for the 2011-2012 academic year.

4) Apply for Work Study (if interested)


If you decide to take a work-study job, you will need to meet with someone in the financial aid
office and complete the appropriate paperwork before starting you working.

5) Receive your award letter


After receipt of the FAFSA results and the above forms, the financial aid office will process your
financial aid and send an award letter detailing your financial aid awards and amounts.
