RULES AND REGULATION

INPATIENT PHARMACY.

1. Floor Stock and After Office Hour medications will be supplied to wards at office hours through
Buku Kimia 21 or Borang Pesanan Ubat and will have to be signed by a trained nurse or medical
officer.

2. Ward staffs are supposed to take the Floor Stock medications. Trained nurse, medical officer or
unit leader have to sign the indent form and return the original copy to the pharmacy.

3. Medications for patients in wards are supplied by ‘unit of use’ through medication chart of
inpatient.

4. Medications for inpatient have to be written and signed by a medical officer.

5. The medical officer will have to fill in List A form to get approval from the chief officers for List
‘A’ drugs.

6. If the patient is admitted after office hour,’ After Office Hour’ medications will be used. If
emergency cases, drugs will be supplied by assistant pharmacist.

7. Ward and unit checking will be done by the pharmacist once every 4 months.

8. To dispose expired drugs, ward or unit has to send the drugs with its drug disposal form. Expired
IV drips can be disposed by the wards.

9. Supply of poison and psychotic drugs:

 Poison drug orders should be made using a poison drug or psychotic drug form which has been
signed by the medical officer or nurse.
 Only a trained nurse or a medical officer is allowed to receive the indent.
 Pharmacist U41 and assistant pharmacist U32 will supply the poison and psychotic drugs to the
wards and units.
 Injections are on ‘exchange basis’
 All the records of poison drug orders should be kept safe and private.

10. Drugs are supplied only if needed by wards or unit.
