International Travel Consent and Acknowledgement

I, { Mark dennison M. Sy}, make oath and state that I am the {FATHER } of:

{Ayeisha Naomi R. Sy}({FEMALE}), born {April 5, 1998} in the Philippines.

1) {Ayeisha Naomi R. Sy} has my consent to travel with {Czarina Razote-Novenario},

(Mother of child).

2) This consent is valid for travel starting on or about {Dec 27, 2010} to { Westin Bay
Shore, Canada} and returning on or about {Jan 1 2011}.

3) I authorize and recommend {Czarina Razote-Novenario}, to make emergency medical

decisions on our behalf, in the event that my child requires emergency medical treatment
at a time or place from which I may not be reached.

4) I authorize and recommend {Czarina Razote-Novenario}, to make arrangements and

determinations for all modes of travel, including itinerary changes as necessary during
this trip.

5) In the absence of {Czarina Razote-Novenario}, should any medical, travel or personal

welfare incident arise, immediate contact shall be made to me{Mark Dennison M. Sy}.
