EMPLOYMENT PASS CHECKLIST

A) PERSONAL FILE

1. Passport copies (applicant and dependent/s)


 Complete set of passport pages including blank pages
 Passport main page must be certified-true by the company
 Upon arrival of applicant/dependent, provide copies of latest entry stamp for submission
purposes

2. Curriculum vitae
 Two copies of applicant's resume, professional accreditation, testimonials etc. All copies must be
certified true and stamped with company stamp.

3. Academic certificate/s (highest)

4. Photographs (applicant and dependent/s)


 Four (4) passport-size photographs with WHITE background

5. Marriage Certificate

6. Birth certificate (of accompanying children)

7. Admission letter from school (for school-going children only)


 Addressed to the Immigration Department, Putrajaya

For renewal of passes/permits:-

8. Latest 3 months salary slips


 Copies must be certified true and stamped with company stamp

9. Latest monthly tax payment receipt


 Copies must be certified true and stamped with company stamp

Notes/Reminders

a. Academic, Marriage and Birth certificate


 Copies of academic, marriage and birth certificate/s must be attested by respective embassy in Malaysia or the
Malaysian Embassy/High Commission in applicant’s home country
 If printed other than in the English Language, please provide translated copy with proper attestation by respective
embassy in Malaysia or the Malaysian Embassy/High Commission in applicant’s home country
 If applicant has step-children, applicant must also present custody letter or letter of adoption with proper
attestation by respective embassy in Malaysia or the Malaysian Embassy/High Commission in applicant’s home
country
 Kindly hand-carry all original certificates which will be required by respective embassy for verification purposes.

b. Passport
 Passport must have at least 6 unused pages before stamping of permit can be done.
 Passport must be valid for at least more than 6 months after term/duration of work permit approved, otherwise
only a portion of the permit approved will be endorsed.

c. Release letter for waiver of 3 months ‘Cooling-Off’ Period (COP) condition


 Provide release letter from previous employer (only applicable to those who has had a work permit with another
Malaysian company)
