Post-term Baby -Extends beyond the week of 42 of gestation (more than 294 days from LMP) -Large for Gestational Age (LGA)- infants who are 4000g or more at birth Causes: y y y y Pregnancy id innacurately detected because of irregular menstrual pattern LMP is unknown Prenatal care was delayed Low levels of estrogen

Fetal Risks of a Post-term baby: y y y y y y y y PPHN (Persistent Pulmonary Hypertension) Birth of an excessively large infant Dysfunctional labor Birth canal trauma Perineal Lacerations Post-partum hemmorhage Infection Placental dysfunction -placental function decreases after 37 wks. -800ml by 40 wks. -400ml by 42 weeks Asphyxia from macrosomia (large baby, with enlarged internal organs) Oligohydramnios Fetal hypoxia related to cord compression MAS (Meconium Aspiration Syndrome) -if not removed at birth can travel down at terminal airways -no signs of depression at birth Hypoglycemia -over utilization of glycogen -due to aphyxiation and physiologic stress Polycythemia -increase in WBC Respiratory distress Post-maturity syndrome

Induction of labor with prostaglandins/oxytocin Vacuum/forceps-assisted birth CS Amnioinfusion to reduce MAS given Intravenously (IV)
