Patient profile: Name: Lampitao, Roldan Age: 15 years old Sex: Male Birth date: May 5, 1996 Address: Benga itogon Benguet Religion: Catholic Attending Physician: Dra. Alma Soriano Admitting Diagnosis: Chronic glumerulonephritis II. History of Present Illness: Patient is diagnose case of kidney problem admitted in this institution due to hematuria and facial edema. Patient was lost to follow-up for 4 years, with no maintained medication. One day prior to admission, patient was complained of headache, frontal area none radiating with 8/10 in severity. There was associated dizziness but no vomiting or nausea. Patient also complained of occasional hypogastric pain. Then was disuria but with occasional hematuria. Persistence of the above condition prompted consult at a private clinic where in an ultrasound was advised. Patient was then brought to this institution for the ultrasound at the same time BUN, creatinine and urinalysis was also requested revealing an elevated creatinine hence was admitted.

V. Drug Study 1. Penicillin Sodium G > Anti microbial, anti biotic, anti infective. >treatment of infection caused by susceptible organism such as congenital syphilis, gonocii, and streptococai (non eterococcal) >

2. Choloramphenicol > Anti microbial, anti biotic, anti infective. > use in widespread antimicrobial bacteriostatic. May be bacterioscidal to species such as H-influenza and nessiria menigitidis > 3. Paracetamol > Antipyretic > Fever reduction and treatment to mild to moderate pain.
