Your charitable organization has proposed a fireworks stand to be located within the jurisdiction of the Sacramento City Fire District. Please provide this District with the following information:

All organizations installing electrical lighting and/or outlets inside the fireworks booth shall obtain a building electrical permit. The electrical permit shall be signed off by an Electrical Inspector prior to the fire inspection. Your organization [ ]WILL [ ]WILL NOT be installing electrical appliances in your booth. Storage method to be used - place an X in the appropriate space:
( ) 1. Fireworks returned to warehouse or supplier each night. ( ) 2. Fireworks stored in stand with watchperson on site no sleeping permitted in the stand or within 25 feet of the stand. Watchperson shall ensure all sales stop at 10 p.m. and shall secure the closure of the booth. ( ) 3. Reinforced, heavy metal, fully enclosed container similar to sawdoc walk-in type drop box, property owners permission required.*

*Note: If option 3 is chosen the storage container may not be located within 25 feet of any permanent structure or other fire exposure. A site plan showing the proposed location of the container in relation to the buildings and access driveways or roadways shall be submitted with this form. A site plan showing the location of your booth on the property is required for all sites Complete this form and return the signed original copy of it to this office no later than May 15, 2012. Place a photocopy of the completed form in the fireworks stand during the sales period.
