PLANNING At the end Of nursing intervention and collaborative, medical management, the patient will regain weight or will not have an additional weight loss. Furthermore, patient will verbalize understanding of necessary caloric intake.

INTERVENTION Assess changes in weight and obtain nutritional history intake. Provide dietary planning related to patients needs. Assist with meals as needed.

RATIONALE To determine severity of the problem.

EVALUATION Goals

To meet the pts nutritional demands

Fatigue may prevent patient from eating and performing ADLs.

Weigh patient daily. To evaluate effectiveness of interventions, and pts response to it. To ensure attainment of optimum nutrition. To treat the infection.

Administer dietary supplements or Total Parenteral Nutrition (TPN) as ordered. Admnister antibiotis for oral candidiasis as ordered.
