PLANNING At the end Of nursing intervention and collaborative, medical management, the patient will regain weight or will not have an additional weight loss. Furthermore, patient will verbalize understanding of necessary caloric intake.
INTERVENTION Assess changes in weight and obtain nutritional history intake. Provide dietary planning related to patients needs. Assist with meals as needed.
EVALUATION Goals
Weigh patient daily. To evaluate effectiveness of interventions, and pts response to it. To ensure attainment of optimum nutrition. To treat the infection.
Administer dietary supplements or Total Parenteral Nutrition (TPN) as ordered. Admnister antibiotis for oral candidiasis as ordered.
ASSESSMENT SUBJECTIVE: Hindi ko na alam ko naong gagawin ko sa buhay ko, as verbalized by the patient. OBJECTIVES: Inability to make decisions in health care Inappropriate use of defense mechanisms Emotional tension Inability to sleep

INTERVENTION Determine pts previous coping patterns. Provide dietary planning related to patients needs. Assess pts perception of current situation.
EVALUATION Goals
Maintain
nonjudgmental attitude in giving care. Encourage pt to participate in own care. Provide opportunity to express feelings.
Irritability
To increase level of self-esteem. To explore pts emotions and provide outlet for stress. To promote
independence.
