MANUAL SKILL: Feeding the Helpless Resident

1. Wash hands. 2. Check the diet card for: name of resident, the diet order, special instructions, and allergies. 3. Make sure the food on try matches the information diet card. 4. Remove tray from food cart, make sure all necessary items are there - silverware, napkin, etc. 5. Take tray to the residents room, or dining area, as quickly as possible to insure that food is served at appropriate temperature. 6. Knock and pause before entering the residents room. 7. Introduce self. 8. Verify the residents identity by checking the armband against the name printed on the diet card. 9. Explain procedure and set mood for a pleasant social experience. 10. Lower the side rail, if permitted. 11. Place the tray on overbed table in front of the resident. 12. If in dining area, remove food items off of tray and place on table. Remove tray. 13. Place a towel or clothing protector. (Avoid calling this bib, it is demeaning to adults.) 14. Remove plate cover. 15. Seat yourself next to the resident at eye level. 16. Arrange food in a convenient manner, butter bread, season food. 17. Describe or show food to the resident before giving it to them. 18. Use adaptive devices as indicated on the care plan, as per individual residents needs. 19. Use straws and thickeners for liquids, as indicated. 20. Offer small portions of solid food (no more than a spoonful) 21. Alternate solids and liquids. 22. Ask the resident what they would like.

23. If the resident has had a stroke, direct food to the unaffected side of mouth. Monitor for pocketing. Monitor for swallowing difficulties; may be at higher risk for aspiration. 24. Place call light within reach. Lower bed to safe position for the resident. Leave room neat. Wash hands. Document. Report abnormal findings to licensed nurse.
