    The ChaCha Account Signature Card/Deposit Account Agreement

    By signing below, I acknowledge that I have applied for an account with First Internet Bank of Indiana (First IB). I have authorized First IB to verify my financial information and data by any necessary means, including obtaining a consumer report by any consumer reporting agency. In accordance with the USA PATRIOT Act of 2001, First IB may request additional information or documents to be submitted in order to verify my identity prior to opening an account. I understand that First IB will retain my application and any supplemental documentation whether or not it is approved. I agree to the terms and conditions of the deposit account and acknowledge receipt of the required disclosures. I also authorize additional deposit accounts I may request in the future via voice, email, or other online communications. Should I request additional deposit accounts, I authorize First IB to verify my financial information and data by any necessary means, including obtaining a consumer report by any consumer reporting agency. I authorize First IB to release my account number and tax identification number to my employer, ChaCha Search Inc., for the sole purpose of making direct deposits into my deposit account.
