The UPHS Isabela Campus is located in Barangay Minante 1, Cauayan City, Isabela, Philippines. It is the latest addition to the growing list of branch campuses which started operation only in the year 2004. It was established for the primary purpose of extending its mission of quality education to Northern Luzon.
It was Mr. Mark Carlo O. Tamayo, grandson of the founder of the University of Perpetual Help System and Mr. Bernard Faustino M. Dy, a childhood friend of the former, who initiated the move to put up an annex of the University in the Province of Isabela. With Mr. Bernard Faustino M. Dy, offering the use of a property from where the school would operate, the partners were inspired with the idea of providing training for more young people who would eventually become productive members of their respective communities. The educational processes, contents and end-results are anchored on its guiding philosophy: Character building is nation building.

The Perpetual help College of Manila opened in 1968 with the key course offering in Nursing. In 1970, the Perpetual Help College in Malasiqui, Pangasinan, was founded to help in the countrys acceleration in health and education development in the rural areas. In 1975, the eldest son of the Tamayos, Antonio, spearheaded the founding of the Perpetual Help College of Rizal in Las Pias. In 1976, the Tamayos established the Perpetual Help college of Laguna and School for Medicine and Allied Health Sciences in Bian, Laguna. On September 10, 1991, the members of the Perpetual Help System- Perpetual Help College of Laguna, the Perpetual School of Laguna Foundation for Medicine and Allied Health Sciences and Perpetual Help College of Rizal entered to accelerate their institutional growth. The Memorandum of Agreement signed in Bian, Laguna, resulted into the creation of an umbrella organization which would 3
College of Information Technology and Engineering implement and monitor common standards in the realization of the system-wide Mission and Vision, Administration, Faculty Academic Programs, Library, Laboratories, Site and Buildings, Instruc-
tional Standards, Accreditation, Research and Extension Outreach Programs. The Perpetual Help System formed the Board of Directors, which serves as its highest governing and policy-making body. It has 15 directors with five each from the three institutions. Dr. Jose G. Tamayo, the Founding Father, was the 1st President and Chairman of the Board, while Dr. Antonio L. Tamayo, President of Perpetual Help College of Rizal, was the Executive Vice President of the System. On January 10, 1993, a top level joint meeting of the board of Trustees and Directors of the Perpetual Help System was convened at the Perpetual Help College of Laguna to pursue a comprehensive and integrated corporate planning process. The objective was to obtain a thorough assessment of the status, conditions and performances of the two campuses to serve as basis for the thrust to apply for a university status. In July of 1994, after the designated committees had submitted their reports assuring the Board of the worthiness and 4
College of Information Technology and Engineering the readiness of the colleges to work towards such goals, the Board agreed to pursue university status through a systems approach. Three years of visitation, evaluation, improvements, and
verifications from the Commission on Higher Education, the Commission en banc, granted the University Status to the Perpetual Help System with campuses in Laguna and Rizal to assume the name University of Perpetual Help System effective Collegiate Year (CY) 1997-1998. In a ceremony held at the UPHS Performing Arts Theatre in Bian, Laguna, on April 20, 1997, CHED Director Amelia Biglete handed to Dr. Jose G. Tamayo the charter granting the Perpetual Help System its University Status. The activity was lovingly dedicated to the pioneering spirit of his late wife, Dr. Josefina Laperal Tamayo. In keeping with its commitment to expand the educational opportunities to answer the needs of more communities, the University of Perpetual Help-GMA Campus was established in June 1997. The operation of this Campus is under the technical control and supervision of the University of Perpetual Help System Laguna. 5

Help System shall continue to give special attention to the promotion and sustenance of the Perpetualite identity in all units as the ultimate end-product of the Perpetualite education.

The University takes the lead role as a catalyst for human resource development, and continues to inculcate values as a way of strengthening the moral fiber of the Filipino individuals proud of their race and prepared for exemplary global participation in the realm of arts, sciences, humanities, and business.

To adopt the Ladder-type of Education, and to contribute to its enrichment towards the concept that each step in the ladder has a definite and significant function to carry out in the delivery of the total spectrum of services.
To promote the progressive development of educational personnel within the University of Perpetual Help System- Isabela Campus.
To adopt and implement a satellite concept of education to solve educational problems in some remote areas of the community.

DAILY ACTIVITIES
In
order
to
finish
our
school, I must acquire two hundred hours (200) of training duties. Our first duty started on the 2nd week of April 2010.

College of Information Technology and Engineering On the first day of our OJT, I was assigned in the office of our Dean together with my classmates who are also having the OJT. We fixed the files of cluster students by category courses. On the second day of my OJT, same task was assigned to me. After we accomplished our task, our Dean assigned us in a respective office in our school. I and my other co-trainee was assigned to the office of Information Technology Services (I.T.S.) wherein they handled us as their OJTs. From there, Maam Ruby was our supervisor while Sir Chester, Sir Jayson and Sir Sandy were guiding, teaching, assist and helping us to accomplish successfully whatever tasks given. First we had to carry out was reformatting some computers in the library and from the computer laboratory and installing application softwares that needed. Theres something errors
when we are reformatting computers but our Sirs had come to it and helping us. After we over and done with reformatting computers another tasks given to us. We cut some bar codes that needed in supply office and then we put on in all supplies in our school. Afterward, we fixed the monitors and CPUs at the I.T.S. office then we put them to the computer laboratory. 16
College of Information Technology and Engineering In my last week of my OJT, our Sirs teach us all about networking. They teach us how to connect some cables and also the RJ45 color coding and to connect to the system and to the internet.
The experience of working and being a trainee is one great chance for students like me. Not all students were given the chance to experience training in an institution with dedicated

College of Information Technology and Engineering employees. Being a trainee for almost 2 months at the Information Technology Services gave me the opportunity to get closer to the employees and staff. I am happy that I have created a good working relationship with my co-trainees and to all the staff. Although I had committed errors in some of the tasks given to me and neglected some instructions, it never became a cause for them not to give me a task. They were all very understanding and always gave instructions clearly. Working under
pressure is one object that I have learned at the I.T.S. I have seen how devoted they were to their profession. As a trainee, I work every task assigned and ensure that I made those tasks more presentable and nice. In my second OJT were a lot of excitement and a lot of knowledge. I am working under pressure but I also enjoying this OJT. Working with the I.T.S. staff bring a lot of pressure to me because they do not only handled the computers for all students and employees and other valuable things like the system of our school, I am grateful that I never had any conflict with them and even with my co-trainees.

I realized that job description should comprise both general areas of responsibilities, in addition to particular assignments. I have developed my public relations by means of consistent dialog in individual in all walks of life.
Attitudes
Staying at the Information Technology Services (I.T.S.) trained me how to work with self-confidence and self-reliance yet under pressure. Always be patient to finish the entire task and always be prompt. Should work with your best and quickly but steer clear of committing errors, pursue each instruction and in so far as possible. Never depart your place of work and be liable enough to finish work on time.

College of Information Technology and Engineering On my second OJT helped me to enhance my communication
skills and have self confidence because at the Information Technology Services office, I have learned to be more familiarized with the hardware of the computer. My working skills also were enhanced. I learned to become innovative and to work through have a valuable to finish the task on time and proficiently, be well-ordered even during rushed hours.
Insight
Learning from experience is oftentimes better than learning without perform. There are many instances that yet a certain individual gain adequate imaginary views and become familiarize with the set values of a certain course. On-the-job training is needed every student to have more skills and knowledge through their courses. To be knowledgeable in instance for having a real and actual job.
