Enhanced Recovery Programme for Elective Primary Hip and Knee Replacements at the Lister Surgicentre

ERP now forms an established surgical pathway in many trusts and its implementation has been accelerated by the QUIPP agenda. It is a multidisciplinary approach that has been shown to improve clinical outcome, patient satisfaction and decreases the length of stay (LOS). It confers both health benefits and cost efficiency savings.
Overview of the Project
A multidisciplinary team from the Lister Surgicentre visited the Norfolk and Norwich University Hospital Trust( NNUH) to observe their established EPR. Various aspects of the NNUH model was adopted and tailored to suite the Lister Surgicentre. An intensive educational programme was launched to promote the merits of the ERP and staff members from the NNUH assisted us in that drive. The key purpose of this was to engage the clinical staff involved in the patient pathway to take ownership of the ERP and empower them to make key decisions in the care of the patient. the longest LOS in the region. A few of the orthopaedic surgeons and anaesthetists proved to be the most resistive with the nurses and therapist being the most cooperative and encouraging. An intensive training and education programme was launched during this transition period with constant feedback to all staff regarding the significant improvements achieved. There are still some surgeons who do not fully comply with the protocol. However, some of the initial sceptics are now championing the ERP.

Progress
The ERP has almost halved the LOS compared to the standard protocol for these operations. Despite the reduction in the LOS with the ERP, there was a slight increase in the DVT and infection rates when it was initially launched. Major non-clinical changes were quickly instituted that resulted in a reduction in these figures which are now within the expected rates for these procedures. The ERP will now be extended to different specialities at the trust.
