TITLE: EDUCATION OF PATIENT

2.1 The patient and/or, when appropriate, his or her family are provided with appropriate education and training to increase knowledge of the patients illness and treatment needs and to learn skills and behaviors that promote recovery and improve function. 2.1.1 The assessment considers cultural and religion practice, emotional barriers, drive and motivation to learn, physical and cognitive limitations, language barriers and the financial implications of care choices. 2.1.2 Instruction on patient rights and responsibilities; 2.1.3 When called for by the age of the patient and length of stay, the facility assesses and provides for patients academic education needs; 2.1.4 The safe and effective use of medication in accordance with legal requirements and patient needs, when applicable; 2.1.5 The safe and effective use of medical equipment, when applicable; 2.1.6 Instruction on potential drug-food interactions and counseling on nutrition intervention and/or modified diets, as appropriate; 2.1.7 Instruction on maintaining oral health, as appropriate; 2.1.8 Instruction in rehabilitation techniques to facilitate adaptation to and/or functional independence in the environment, if needed; 2.1.9 Access to available community resources, if needed. 2.2 The patient and/or, when appropriate, his or her family are provided with the specific knowledge and/or skills required to meet the patients ongoing healthcare needs. Such instruction is presented in ways understandable to the patient and/or his or her family and includes, but is not limited to, the patients and familys responsibilities in the patients care. 2.3 With due regard for privacy, the facility teaches and helps patients maintain good standards for personal hygiene and grooming, including bathing, brushing teeth, caring for hair and nails and using the toilet.

4.1 Physician 4.2 Registered nurse 5. PROCEDURE 5.1 Upon admission, the licensed nurse will begin the nursing process for patient education as follows: assess learning needs and learning capabilities of patient, establish teaching plan based on goals and objectives of patient education for the individual, implement the teaching plan and evaluate learners response. 5.2 Upon admission, the staff nurse will orient the patient to the environment-physical, patient rights and responsibilities, facility staff, safety issues and other pertinent issues. 5.3 Teaching will be initiated when the patients condition permits. Teaching will be modified, as needed, to meet individual needs; i.e., language barrier, physical limitations. The family and/or significant others will be included in patient education as frequently as possible, when appropriate. 5.4 Education will include information, as necessary, about patient responsibilities in his/her own care, including, but not limited to responsibilities for: 5.4.1 Providing information 5.4.2 Asking questions and obtaining information about his/her care 5.4.3 Following instructions 5.4.4 Accepting the consequences of not following care and/or treatment 5.4.5 Following rules and regulations regarding care and conduct 5.4.6 Showing respect and consideration for other patients/facility personnel and property 5.4.7 Meeting financial considerations 5.5 Relevant written materials (approved by patients physician) including a discharge instruction sheet will be given to the patient by a licensed nurse. The patient will sign the discharge instruction sheet after the patient education has been completed and he/she will retain a copy of his/her discharge instructions for home use. 5.6 Such patient education includes instruction in the specific knowledge and/or skills needed by the patient and/or where appropriate, his/her significant others, to meet the patients ongoing healthcare needs including: 5.6.1 Safe and effective use of medication 5.6.2 Safe and effective use of medical equipment 5.6.3 Potential drug-food interactions and counseling on medical diets 5.6.4 Maintaining oral health 5.6.5 Instructions about any follow-up care and how to obtain care
