Dear Parents

In previous years, we asked you to complete an admission form informing us of the necessary details
which need to be held on the school database. For most children this information never changes and
therefore it is no longer necessary to keep asking you to complete duplicate forms. However if your
circumstances do change, for example if you move house, change your phone numbers or more
importantly, your emergency contact details, then it is vital that you inform us immediately.

Attached is a ‘Change of Details’ form. If you are aware that the personal information we hold on
your child is no longer current, then please return the form to school giving the updated information.

It is your responsibility to ensure that the information stored is correct; we can only work on what
you tell us, so please continue to update us throughout the year if your circumstances change. The
information is shared with the Local Government office, who make several requests for us to share
the statistical data.

Throughout the year the children are often involved in events which celebrate their own or the school
successes. Often there are photo opportunities to promote this success which can either be
published or displayed around the school. Would you kindly sign the attached form indicating that you
are happy for your child to be photographed for this purpose.

On odd occasions we walk out of school around the community as part of our topic for the term.
Would you also sign the consent form indicating that you happy for your child to participate in these
activities when necessary.

Lastly would you supply us with information requested by the government for statistical purposes,
indicating how your child arrives at school each day and their arrangements for lunchtime.
