What is UTT-UA After-school Program? The UT Tyler University Academy is an award-winning After-School Program offered at area schools to provide students with enrichment activities that are engaging, academic and fun. UTT-UA is now offering this program at the 3 Innovation Academy Campuses: located in Tyler, Palestine and Longview. What does the UTT-UA After-school Program offer IA students? This program provides every participating student the opportunity to: Enjoy a sack lunch they bring from home (please see attached Acceptable Snack list to avoid sending foods that others may be harmful to others who have peanut and nut allergies). Complete their homework in an environment where assistance and encouragement are actively provided. Receive intervention based on identified needs. Participate in STEM/PBL based activities such as robotics, coding/programming, etc. Participate in interest-based enrichment Activities/Programs (sports/recreational/arts/social/etc.). Develop College and Career Readiness, Cross Curricular Skills, and interest/belief/vision. How can my child attend the UTT-UA After-school Program at the IA campuses? All students who would like to attend the After-School Enrichment program should be registered through the on-line registration form ahead of time. How do I register my student (child) for the UTT-UA After-school Program at the IA campuses? On-line registration can be found at the following site: https://adobeformscentral.com/?f=RpbWbayxA3Dxl6lrK-WZ3Q Select the campus you are interested in, select the days you would like your student to attend: 5-days per week, or any specific weekdays and fill out the remaining form. What is the cost for attending the UTT-UA After-School Enrichment Program? Session 1 Cost: $10/child/weekday Begins at 12:30 pm and children must be picked up by 3:30 pm. Session 2 Cost: $10/child/weekday Begins at 3:30 pm and children must be picked up by 5:30 pm. Register for both sessions and save! Cost: $15/child/weekday Begins at 12:30 pm and children must be picked up by 5:30 pm. Pick-up times are promptly observed and late pick-up will result in additional $10 per hour of tardiness. IA reserves the right to disenroll any student whose parent is habitually late in picking the student up at the end of the scheduled programming day. Where will UTT-UA conduct the After-School Enrichment program? The program will be conducted at the respective school campus, in a designated area of the campus that is safe and monitored by the UTT-UA staff and teachers. Each student must be signed out when picked up by a designated adult. The students will not be allowed to leave the premises without the designated adult signing them out. 1

Can we submit the registration form in person? No, only on-line registration is being accepted at this time. We are building a database of information for each student who is attending this program and need all the contact information, medical history, parent or guardian contact, permission to pick the child up, and other necessary information in an electronic form for quick and accurate access as well as for billing purposes. Therefore we need each parent / guardian to register their student(s) through the on-line form that can be found at: https://adobeformscentral.com/?f=RpbWbayxA3Dxl6lrK-WZ3Q I have registered my child for this program and need to make changes to the registration details such as days and sessions. How can I do that? Making updates to your registration is not a problem. Please send an email to uttua@ingenuitycenter.org . In the email, please indicate your name, your students name, the IA campus where your student is attending, your contact information, and the exact details about the information you would like to update. We will gladly make the change and confirm that with you via email. I have already registered my student and would like to make a change to the number of days or sessions. Will there be a penalty to make this change? No, there is no penalty to increase or decrease the number of days your student attends the program. We request that you give us a notice as far in advance as possible so that we have staffed the program well to meet the needs of the students. A 24-hour in advance notice is required if you would like to add or drop back in your students involvement in the program. The change can be permanent or temporary, but you must send an email to uttua@ingenuitycenter.org . In the email, please indicate your name, your students name, the IA campus where your student is attending, your contact information, and the exact details about the information you would like to update. We will gladly make the change and confirm that with you. Are there any scholarships / discounts / TX Workforce Commission Contract available for the UTT-UA Afterschool Program? The UTT-UA After-school Program at the Innovation Academy Campuses does not offer any scholarships / discounts / TX Workforce Commission contracts options. A public announcement will be made if such an opportunity becomes available in the future. If my child is ill, do I still pay for the day not attended? If your child is ill, when you contact your school to report the absence, please ask them to notify the UTT-UA After-school program that your child is ill and will not be attending that day. Additionally, please send an email to the UTT-UA program uttua@ingenuitycenter.org to report the absence due to the illness, as early in the day as possible. If you pick your child up early due to an illness, please notify your schools administrative secretary that your child will not be attending the After-school Program and follow up with an email to: uttua@ingenuitycenter.org If we do not receive any communications from you regarding your childs absence due to an illness, then the charges for the After-school Program will be applicable and will show up on your bill. Please give us advance notice of absence due to planned family events as well. If your child is registered and scheduled to attend and does not show up, you will be charged for the day based upon the sessions your student is registered to attend, unless we have received and acknowledged information about the absence. 2

How will the billing work? Will there be weekly or monthly bills? Can we pay on-line? Are checks acceptable? Please calculate your payment based on the days you have registered your student to attend and the sessions (session 1 or both sessions). You can pay by cash, check or money order, payable to the Ingenuity Center. Advance payments are due in 2-week or 4-week increments whichever is convenient for you. At this time, we are not able to accept payments via credit cards, or online, but we hope to offer that option in the very near future. For drop-in and as needed attendance, payments are due on the day of attendance.

Please note: All resources and staffing are allocated based on the student registration; therefore, we must charge based on your registration preferences (days and sessions). If you do not use the days and times your student is registered for, then we are unable to give you credit for those days because we are expecting your student and have already assigned the staffing to support the need. Request to make changes to your students schedule (due to doctor appointments and family travel) will be honored if made at least 24 hours in advance sooner if possible. We sincerely appreciate your cooperation and understanding regarding this matter.
