Primary Care Appointments

Problem Statement

TMFHS Emergency Care Center , Hospitalists, and Specialists have identified that it is difficult to
schedule both new and established Smith County and regional patients in a timely manner into a
primary care appointment for appropriate follow up, or to establish care. This creates significant
patient satisfaction issues, may lead to increased readmissions to the hospital, and/ or unnecessary
return visits to the ER for simple follow up appointments. We have many providers at Trinity Clinic
Douglas, and in all clinics in the region that are actively seeking new patients.

Solution

1. Use a streamlined process for scheduling patients into open appointments with primary care
providers and midlevel providers at primary care clinics that work closely with attending
physicians.
2. Routinely publish updated list of Trinity Clinic providers that are building practices and are actively
seeking new patient.
3 . Simplify a list of phone numbers that ward clerks, nursing staff and ECC staff can use to schedule
patients into primary care appointments.

Process Effective Immediately

1. ECC, Hospitalists, Specialty Offices and Telecare Plus will be given a list of primary care
physicians and midlevels providers with capacity for new patients.

2. To schedule established patients with providers for ECC or hospital follow up appointments, the
ECC and ward clerks will utilize P1C or a central call number into Douglas clinic during clinic hours
identified for this purpose only. This number will be answered promptly. These patients will be
scheduled in to their primary care doctor or a midlevel schedule within 2-7 days, depending on the
specified necessity as dictated by the discharging provider. The midlevel provider will then work with
clinic staff to schedule subsequent appointments with patient’s primary care physician. After clinic
hours, Telecare Plus will be utilized for scheduling purposes.

3. For patients that live in the region, please call primary care clinic closest to patient, or where they
are established. Utilize Telecare Plus to schedule patients after hours.

4. For ER follow up patients, please fax the ER patient treatment information to Douglas Call center
Fax # 510-8899, and it will be given to our providers to eliminate unnecessary wasted time spent
looking for records.

the clinics during their open hours, or schedule into the clinic directly
