This will allow the coordinator to see in person the trainee wherea"outs and status in the compan he is working. Spot checking can also "e done through telephone call of the compan.s contact person. OJT &'it Interview ( this inter!iew is spearheaded " the immediate super!isor of the OJT trainee. This is done to assess the trainee total performance. Although) it is again the prerogati!e of the compan to conduct such e0it inter!iew. Performance &valuation ( This is a set of questions or a checklist for the immediate super!isor to answer with. This document will "e the ma+or "asis for e!aluation of the trainee during the period of his practicum. (ebriefing ( This will "e conducted " the OJT coordinator within the school premises. A consultation of what ha!e transpired in the time the trainee was assigned to a compan is discussed. This will gi!e to the coordinator an idea of the real status of e!er trainee if the complied the required num"er of hours or need for an e0tension. Post-OJT Requirements ( This is where the su"mission of all the OJT required documents occurs. The OJT requirements and guidelines will "e full o"ser!ed " the trainee and all these su"mitted documents will "e checked and e!aluated " the OJT coordinator for proper filling and safe keeping. Issuance of OJT )rade ( After e!aluating that the trainee has compl all the documentation) the OJT coordinator will now issue*release the OJT grade "ased on the trainee performance.
