The foundation of Person Centered Care is based on the !or) of Professor Tom .it!ood !ho
headed the /radford Dementia &roup in the United .ingdom from 0112 to 0113'

The practice of Person Centered Care begins b, %oo)ing at the individua% as a person !hose
dementia forms on%, part of his tota% identit,' The %arger part of this individua% continues to be the
possession of his or her o!n uni+ueness( the retention of the need for se%f-recognition and
identit, as !e%% as the preservation of their abi%it, to act and respond to the environment that
surrounds them'
